Community Based Services

Community Support

Community Support is designed to support children with severe emotional disturbance in their home and community. Community Support works to support individuals through:

  • Referral - Support families in identifying other needs and making appropriate referalls (i.e. Outpatient Therapy and Respite)
  • Assessment - Children work with staff to assure up to date and accurate assessments
  • Individual Case Planning - We work to develop comprehensive individual plans of care that meet the complex needs of individuals served
  • Service Coordination - Staff connect with schools, PCP's and other community partners to identify and secure services needed to meet goals outlined in their plan of care
  • Transition Planning - Work with families to allow for a thoughtful and effective transition

Community Support Specialists work with community partners to help ensure success for clients and families. Community Support Specialists meet with children both individually and with care givers to meet thier goals. Some frameworks used by Community Support Specialist include:

  • ARC
  • Motivational Interviewing
  • Solution Focused Treatment

Respite Services

A treatment and support service available for the families of children who are experiencing, or are at risk for experiencing, severe emotional disturbance. Respite is available as a planned service or emergency.
