Have Questions About NCSS?

Out of respect for each individual, NCSS believes that it is important to speak directly to an adult about scheduling their own appointment, so that we can make sure that they really want treatment, and to determine any personal preferences. A child’s legal guardian may, of course, schedule an appointment for the child.

NCSS provides a comprehensive range of health and human services designed to support individuals and families at every stage of life. Our programs focus on promoting mental health, emotional well-being, personal growth, independence, and community connection through compassionate, person-centered care.


We serve children, adolescents, adults, and older adults through a variety of community-based programs that address mental health needs, developmental disabilities, family support, early childhood development, crisis intervention, education, housing, employment, and community living. Our team works alongside individuals and families to build on strengths, navigate challenges, and access the resources and supports needed to achieve their goals.

To request a copy of your records from NCSS, you will need to complete an Authorization to Disclose or Request for Access form, depending on who you would like to receive your records. This ensures NCSS is clear about what information is being requested and that your information is shared securely and in accordance with privacy regulations.


You can request records by contacting your NCSS program directly or by calling 802-524-6554, and staff will help guide you through the process or direct you to the appropriate department. In most cases, you may be asked to provide written authorization and a valid form of identification before records can be released.

Once the request is received, records are processed as promptly as possible, though timing may vary depending on the type and volume of information requested. If there are any questions or special circumstances, the NCSS team will work with you to ensure the request is completed appropriately and in line with applicable privacy laws.
