Women's Health Initiative - Social Work Care Coordinator

Come join our dynamic Team while contributing to an exciting new initiative that will directly impact the health of women in our community. 

Activities

  • Ensure women’s health providers have the resources they need to help women be well, avoid unintended pregnancies and build thriving families.
  • Consult with primary care and specialist providers.
  • Implement screening protocols for mental health & substance use disorders.
  • Provide short-term, solution-focused therapy while linking patients with necessary services.

Requirements

  • Licensed Clinical Social Worker is preferred, however other licensed individuals will be considered; Master’s in Social Work is acceptable with at least 1 year of experience and a clear plan for supervision toward licensure.
  • Ability to use mental health and medical social work experience to deliver high-quality patient care.
  • Ability to work within a multidisciplinary team.
