Resource Coordinator

We are seeking an individual who is interested in providing individualized case management support to families of children who have developmental disabilities. Ideal candidate should be patient, mature, responsible, able to work independently and as a part of a larger treatment team.

Primary responsibilities include:

  • Comprehensive care coordination with providers, families, guardians, local service agencies, medical professionals, and other necessary parties.
  • Collaborating with Community Partners to ensure families and their children access appropriate supports
  • Individualized treatment planning to include referrals, advocacy, and monitoring.
