Residential Service Coordinator

The Community Rehabilitation and Treatment Team is looking for a strong leader and team player to provide administrative and clinical oversight of a transitional living program for adults with severe and persistent mental illness. This person is responsible for managing daily operations of the residence and coordination of treatment for all residents.

Duties include:

  • Developing and implementing an Individual Plan of Care (IPC) for assigned clients on case load
  • Coordinating services to support individuals within the residence
  • Linking residents to community resources
  • Working with residents to enhance independent living skills
  • Managing daily operations of the residential setting

A Bachelor’s Degree with experience working with individuals with mental illness preferred but willing to train the right candidate.
