Reach Up Case Manager

Seeking a full-time Case Manager to assist individuals/families who access temporary assistance. This position helps families to move towards self-sufficiency through motivation and encouragement. Case Managers screen, assess, and determine eligibility for participants applying for the Reach Up program; develop appropriate case plan with participants that outline steps, tasks, and timeframes for achievement of employment goals; provide regularly scheduled home or office visits to assist and motivate participants in implementing plans and meeting federally established work participation rates within prescribed timeframes; monitor progress of plans, revises plans, and take action when appropriate; coordinate activities with clients in using agency services and other community resources; work closely with state agencies, providing services and necessary documentation as appropriate; assure timely and appropriate service delivery; inform participants of and advocates for available community services and offer supportive counseling to assist participant in resolving life issues interfering with employment goals; compile records and prepares reports as required; document in a timely and accurately in computer system and case records with federal verification. There are approximately 40 families/single parents within the Reach Up program.

Case management experience necessary. Dependable vehicle, proof of insurance and a clean, valid driver’s license are required for necessary travel. Must be able to interpret federal and state rules surrounding this program, be able to provide a holistic approach and to adapt and communicate effectively in a challenging environment.
Bachelors Degree, preferably in Human Services or Social Work, required
