Prevention and Stabilization Program Case Manager

The NCSS Adolescent Services Team is Seeking a Case Manager for our Prevention and Stabilization Services Program.

Duties include:

  • Applying program support to juveniles with documented substance use or servere emotional disturbance and their families.
  • Developing case plans.
  • Facilitating access to services and monitoring participant progress
  • Following program recommendations.
  • Assessing program and participant needs.
  • Ensuring a continuum of service delivery from advocacy to treatment.
  • Monitoring the effectiveness of services and communicating client’s program progress.
  • Comparing data and compiling reports.

Candidate will participate in treatment team meetings and trainings or conferences related to program. Candidates must be well organized and posses a high level of administrative skills. Flexible hours are necessary with evening and limited weekend hours.  Bachelor's degree in the Human Services field is required.
