Pediatric Social Work Care Coordinator

Do you enjoy working with children and their families? Are you looking for supervision hours toward licensure? Contribute to an exciting new initiative designed to increase access and outcomes for individuals served in their primary care setting. Social Work Care Coordinators have the opportunity to work closely with an interdisciplinary team serving Northwestern Vermont. This position is primarily located in the Enosburg Falls area.

Duties include:

  • Consulting with primary care pediatric practitioners
  • Implementing screenings for social determinants, mental health, and other condition sets
  • Being a referral source for local services
  • Coordinating care for pediatrics and their families
  • Providing short-term, solution-focused therapy

Requirements include:

  • License eligible Master’s degree in a mental health field
  • Experience in a mental health and medical social work setting
  • Strong interpersonal, communication, and organizational skills
