Intensive Case Manager

The Community Rehabilitation and Treatment Team is looking for the right team member to provide intensive case management and medication management and/or medication deliveries to individuals with severe and persistent mental illness. Responsibilities include providing assistance with activities of daily living, money management, social and interpersonal skill development, advocacy and linkage to community resources, including making home visits. Position requires the ability to work flexible schedule, including evenings and weekend rotation. Must have reliable transportation and a clean driving record with the ability to transport clients. Good attitude and moderate computer skills is a must! Case management experience is a plus but willing to train the right individual. Bachelor’s Degree preferred, however an Associate’s Degree in the Human Service field and/or relevant experience will be considered.
