Enhanced MAT Care Coordinator* (Sign On Bonus)

Northwestern Counseling & Support Services is seeking a candidate to join our dynamic and supportive team in piloting an enhanced role which connects Medication Assisted Treatment (MAT) with Integrated Health Care Coordination. This position is ideal for a person with co-occurring treatment experience and licensure. This position is eligible for a sign-on bonus to be paid out at a designated time. Duties include:

• Consulting with primary care and MAT providers
• Implementing screenings for social determinants, mental health, and substance-use disorders
• Connecting patients with appropriate services and resources
• Providing short-term, solution-focused therapy
• Working as part of a multidisciplinary team

Requirements include:

• License eligible Master’s Degree and/or LADC
• The ability to deliver high-quality patient care
• Experience in mental health and medical social work settings
• Strong interpersonal, communication, and organizational skills
