We are seeking an individual with a Bachelor's degree in Social Work or other relevant field for a blended role of Intake Screener and Mobile Outreach Case Manager. Essential qualities include attention to detail and the ability to gather accurate and comprehensive data. Candidates should demonstrate the skills necessary to work with individuals who have difficulties engaging in care, support clients with problem solving, and collaborate well with other providers.
