Services Coordinator / Developmental Services

The Transitional Support Team is seeking compassionate and energetic candidates to provide case management services for individuals in Franklin & Grand Isle Counties.  Ideal candidates should have a positive attitude; strong and supportive presence; communicate effectively verbally and in writing; have good computer skills; willingness to provide occasional transportation, attend medical appointments and home visits; and the ability to work in a fast paced team environment, maintain objectivity, think critically, and manage stressful situations in a non-reactive manner.  Position is very autonomous and provides a tremendous opportunity for flexibility.  Candidate must possess a Bachelor’s Degree in a human service field.  Prior experience working with people who have developmental disabilities/behavioral health conditions is preferred.  Candidates must have reliable transportation and clear background screenings.

Developmental Services
