Position	Intensive Case Manager
Description	The Community Rehabilitation and Treatment Team is looking for the right team member to provide intensive case management and medication management and/or medication deliveries to individuals with severe and persistent mental illness. Responsibilities include providing assistance with activities of daily living, money management, social and interpersonal skill development, advocacy and linkage to community resources, includes making home visits. Good attitude is a must! Case management experience is a plus but we are willing to train the right individual.

Skills Required	Position requires the ability to work a flexible schedule, including evenings and weekend rotation and moderate computer skills. Must have reliable transportation and a clean driving record with the ability to provide transportation to clients.
Education	Bachelor’s Degree preferred, however an Associate’s degree in the Human Service field and/or relevant experience will be considered.
